Cannon wise, Zero has only died twice, once in MMX1 and once in MMX5.
(He was put into stasis in the time period between MMX and MMZ so that doesn't fully count as a Death and if you "die" as him in MMX3 he's just put out of commision for battling (Ex. His power systems get busted and cant power his weapons))
